Continuous Monitoring:

  • Regularly monitor all active support cases for signs of distress, mental health challenges, or self-harming behaviour.
  • Stay vigilant for changes in behaviour, circumstances, or external factors that may indicate an increased risk of harm.

Thorough Assessment:

  • Conduct ongoing assessments to evaluate the presence and severity of potential harm risks within each support case.
  • Utilise a combination of observation, dialogue, and structured assessment tools to gather relevant information.

Risk Identification:

  • Identify specific risk factors that may contribute to harm, including but not limited to suicidal ideation, previous suicide attempts, substance misuse, or social isolation.
  • Consider the individual's personal history, current circumstances, and any external stressors that may impact their safety.

Protective Factors:

  • Recognise and reinforce protective factors that promote resilience and mitigate risk, such as access to supportive relationships, coping skills, and positive life experiences.
  • Empower individuals to utilise their strengths and resources in times of distress to enhance their safety and wellbeing.

Collaborative Approach:

  • Engage in collaborative discussions with the individual, their support network, and relevant professionals to gain a comprehensive understanding of the situation.
  • Share information and coordinate efforts to ensure a unified and holistic approach to risk assessment and management.
