After each therapy session, we ask you to fill out a Session Registration Form. It’s quick but important — it helps us track patient progress, safeguard wellbeing, and maintain high-quality care across the program. Find the Session Registration Form here.
What the form includes:
- Date and duration of the session
- Type and location of the session (online, in-person, etc.)
- Any safeguarding concerns
- A short summary of what was discussed
These notes are not your detailed clinical records but help us understand the support being given. If any issue arises later, your notes provide a clear and confidential log of the care you’ve provided.
You can also access all your previously registered sessions anytime through the Volunteer Therapist Platform for your own records and follow-up.
